Documents required for Marine claims:
 

1.

Copy of Insurance Policy and/or Marine Risk Note;
2. Written Notice of Claim to the broker or Insurance Company of North America stating the following:
  a. Detailed Circumstances of the Loss
  b. Computation of Claimed Amount
3. Original or Duplicate Copy of Bill of Lading
4. Original or Duplicate Copy of Commercial Invoice
5. Original or Duplicate Copy of Packing List
6. Receipted Copy of Claims against Negligent Bailees
7. Discharging Tally Sheets
8. Delivery Receipts
9. Warehouse Receiving Report
10. Stripping Report, for containerizes shipments
11. Survey Report, if any
 

Additional Requirements are needed for loss or damage due to the following:
 

1.

Damaged Cargoes (e.g. Denting, Scratching)
  a. Equipment Interchange Report
  b. Bad Order Certificate
2. Missing/Short Delivery
  a. Short landed Certificate
  b. Equipment Interchange Report
  c. Loading Survey Report
3. Wetting/Contamination
  a. Equipment interchange Report
  b. Laboratory Analysis Report
4. Vehicular Accident/ Robbery/ Hi-jacking
  a. Police Report within 24 hours from the time of incident
  b. O.R. and C.R. of Truck Conveyor
  c. Driver’s License and O.R.
  d. Statements of Driver and Helper
